ISN Support Services Brokerage
Assuring a voice and a choice for adults with developmental disabilities.
A partnership of Catholic Community Services and Chehalem Youth & Family Services since 1997, the Integrated Services Network (ISN) Brokerage is a state-funded program that offers adults with developmental disabilities the opportunity to live in the community and reach for their dreams.
Under this program, customers are viewed as citizens with the rights and responsibilities of self-determination, choice and ownership. Those served are offered an array of services and providers, such as employment supports, respite care, community inclusion activities, transportation, in-home support, environmental adaptations, or other supports that assist them to live, work and recreate in their community.
Many ISN Brokerage customers thrive on leadership training which prepares them to advocate on their own behalf regarding affordable housing, employment, education, transportation, and health insurance issues.
